DK PRODUCTIONS GROUP, LLC AGREEMENT

Thank you for choosing consulting services from the DK Productions Group! We are not your average consulting firm! It is our pleasure to sharpen you and help you reach your desired goals.

Clients who are retaining the services of DK Productions' Principal Consultant David Price should complete this form prior to any consultation services commencing. By completing this form, you are agreeing to all policies and procedures as described. You may cancel at any time and you are only contracting us for the agreed time. You are not under a month to month obligation.

Billing for services will be done by our office and a valid card must be kept on file. Please list the desired method of payment you would like to use for your services. Payments will be processed for scheduled services 24 hours prior to service being rendered. Services canceled 12 hours prior to scheduled time will receive a credit. Services not canceled will incur a fee of $50 and any remaining credit will be applied to next scheduled service. *

A cancellation fee of $50 will be assessed if I do not cancel appointments 12 hours prior to scheduled appointment time. Cancellations must come in form of email, text or voicemail only with a time stamp verification. Fee will be taken from the amount of the session that was pre-paid. Balance of session fee may be used towards the next session.
